Requesting Reimbursement for Costs - Grant Application

NOTE: Desert Cares Foundation, Inc. does not provide funding to individuals. We only work with health and human services agencies.

To be eligible to be reimbursed for costs for services and supplies, an agency must complete a grant application, submit proof of business entity documentation, and submit the costs associated with the services they provide.  Once the grant application is approved by Desert Cares Foundation, a cost-reimbursement contract will be executed with the selected providers.  

Each provider submitting reimbursement requests must provide documentation of the patient's financial hardship and cost documentation (such as the invoice for purchasing meal replacement shakes, payroll information for staff that provided care to the patient, receipts for equipment purchased or rented, etc.).
